WILDLIFE INTAKE FORM — REQUIRED BY LDWF

Thank you for your care and concern for wildlife. Sundown Inc. is a state-licensed wildlife rehabilitator for Acadiana, permitted by the Louisiana Department of Wildlife and Fisheries (LDWF) to rehabilitate skunks, raccoons, and squirrels.

Per LDWF regulations, this intake form is MANDATORY. If we have agreed to accept an orphaned or injured animal from you, you MUST complete and submit this form before the time of drop-off/transfer. Intake cannot be completed without it.
